What is SHARE? 

SHARE for Dementia is an evidence-based care-planning program that empowers adults with early to mid stage dementia and their families to get the most out of today while planning for tomorrow.
Developed by the Benjamin Rose Institute, SHARE is designed to engage both a person with early to mid stage dementia and their care partner in discussions about symptoms, communication, care values and preferences, healthy activities, and planning for the future.

What is 1:1 Care Navigation? 

Trained Care Navigators, also known as coaches, assist in helping your family and your loved one who is experiencing memory loss with developing individualized short and long term care plans which include Financial, Legal, and Healthcare assistance.
Care Navigators support recommended treatment and activity plans by helping clients to maximize their participation.  Through consistent contact, Coaches build trusting relationships, actively respond to emergent needs and seek solutions tailored to the families interests and needs.
